Insurance participation is subject to credentialing, contracting, and effective dates established by each health plan. Coverage and out-of-pocket costs vary by individual plan.

Payment is due at the time of service. A limited number of reduced-fee appointments may be available based on financial need and practice capacity.
Clients who are uninsured or choose not to use insurance may request a Good Faith Estimate of expected charges before services begin.
